Printable Opened-Vial Log

A one-page bench log for multi-use vials. Each vial gets a line the moment it is first opened; the discard date is 28 days after opening by convention (or the period stated on the label, if different), and the vial is discarded on that date even if liquid remains. Instructions. Fill in the line at the moment the flip-off cap is removed, not later. Write the same date opened and discard date on the vial flag label. Check the log weekly for vials at or past their discard date and discard them, recording the date and initials. A vial that shows cloudiness, particles or color is discarded immediately regardless of date, with a note in the Location column. Keep completed sheets with the reagent records.
